Here’s how it works:
Fill out the basic information to the right and please include all relevant information for you/your loved one to the best of your “current” knowledge (we can adjust and change as your needs or circumstances may/may not change over time).
Please Remember: the person at the end-of-life is in full control of this journey and how much/how little the doula will be involved every step of the way.
You will then receive an email from swansongsdoula@gmail.com with availability for a real-time phone conversation/consultation. Following the consultation, we will schedule a visit to discuss a care plan together and make sure the person on their end-of-life journey feels their needs are being met and this is a good fit for their end-of-life wishes.
